Our Occupational Therapists can now provide various services to adult clients including Functional Capacity Assessments (FCA), Home and Living assessment and reports (including Supported Independent Living and Specialist Disability Accommodation (SDA), Assistive Technology and Home Modification scripting and applications plus more.

Services by Age

Occupational Therapy for Young Adults (18-26)

During the transition into adulthood, we understand the importance of fostering independence and developing essential life skills. Our Occupational Therapists offer personalised interventions to support young adults in achieving their goals, whether it's through vocational training, education support, or enhancing daily living skills. We can also assist young adults with exploring options for supported independent living including exploring and applying for Home and Living Supports available under the NDIS.

Occupational Therapy for Later Adulthood (27-64)

As individuals navigate the responsibilities and challenges of adulthood, our services aim to promote independence and enhance quality of life. Whether managing chronic conditions such as arthritis, living with a serious health condition, disability or injuries, our Occupational Therapists provide interventions tailored to each individual's needs, including both low cost Adaptive Equipment and high-cost Assistive Technology recommendations, Home Modifications, and support with activities of daily living.

Occupational Therapy for Seniors (65+)

We recognise the importance of ageing gracefully and maintaining optimal function and independence in later life. Our Occupational Therapists specialise in addressing the unique needs of seniors, including mobility issues, cognitive decline, and age-related conditions. We strive to empower seniors to live life to the fullest through personalised interventions including falls prevention management and Assistive Technology solutions.

Assistive Technology & Equipment
Our Occupational Therapy (OT) services encompass comprehensive assessments and personalised equipment prescriptions, ensuring individuals achieve optimal independence and functionality in their daily lives.
Occupational Therapists assess functional abilities and limitations, recommending Assistive Technology to enhance daily activities for people of all ages. Belong Health’s specialised OTs provide tailored equipment solutions that empower individuals and simplify everyday tasks.

Occupational Therapy Services for Functional Capacity Assessments (FCA)

Our Occupational Therapists can complete a Functional Capacity Assessment at individual’s homes throughout Perth and Mandurah surrounds, in our clinic in Falcon and via Telehealth (where appropriate). As NDIS registered Occupational Therapists, we are able to work with NDIS participants with all funding types.

NDIS Home Modifications

Our Occupational Therapists (OTs) specialise in facilitating minor and complex NDIS Home Modifications, ensuring individuals' homes are safe, accessible, and conducive to their independence.
Our OT's assess each situation on a case-by-case basis to determine the most appropriate interventions. We collaborate closely with clients, their families, and other healthcare professionals to ensure that modifications meet individual needs and promote safety and independence.

Specialist Disability Accommodation (SDA) and Supported Independent Living (SIL) Assessments & Reports

We specialise in comprehensive home and living assessments including for Supported Independent Living (SIL) and Specialist Disability Accommodation (SDA), offering tailored solutions to meet the unique needs of individuals seeking greater independence and suitable housing.

Specialised Interventions:
Our team has expertise in working with a wide range of disabilities and conditions, including:

Physical disabilities such as but not limited to; amputations, arthritis, cerebral palsy, and spina bifida.

Neurological conditions including but not limited to; Multiple Sclerosis (MS), Motor Neuron Disease (MND), Muscular Dystrophy (MD), Parkinson's Disease, Post-Stroke rehabilitation, Acquired Brain Injuries and Traumatic Brain Injuries (ABI/TBI).

Various other diagnoses including; Autism Spectrum Disorder (ASD), Intellectual Disabilities and Mental Health Conditions.

Conducting in-depth assessments and producing reports for various purposes, including NDIS (National Disability Insurance Scheme) Assistive Technology and Home Modification applications, Functional Capacity Assessments (FCA), Supported Independent Living (SIL) or Specialist Disability Accommodation (SDA). We can also provide support with NDIS change of circumstances and funding needs re-assessments.

We conduct comprehensive low cost and high cost assistive technology and home modification assessments and provide support letters and written applications for these.

Let's Work Together

Referrals to Belong Health
We are Medicare and Private Health fund registered for Occupational Therapy and Speech Pathology.

EPC/Team care arrangements/Chronic disease (CDM) or GP management plans and self-referrals are all acceptable.

WA NDIS Registered Provider

Medicare
You can get a referral from your/your child’s GP for OT on a Chronic Disease Management (CDM) plan this gives you 5 sessions for OT per calendar year with a $53 rebate available on each session or a Helping Children with Autism/PDD/ or an eligible disability package (20 sessions between allied health professionals, as set up by a Paediatrician/Psychiatrist) which gives your child under 16 years a $75 rebate per session for the 20 sessions, you can use up to 10 sessions per Health Professional (up to 10 for OT) and you will need a separate referral letter from your child’s Paediatrician for each Allied Health Professional you are seeking services from.

Private Health
Your Private Health fund may cover up to 75% of the cost of OT sessions if this is included in your cover, be sure to call them or check your policy for details.

No referral or self referral
You do not require a referral for accessing OT

NDIS Registered Provider
We are an NDIS Registered Provider
Whether you or your child have NDIS funding that is NDIS Managed, Third Party Plan Managed or you are Self-Managing your funds, you are able to use your funding to cover the full cost of therapy services plus travel costs. You will need provide us with your/your child’s NDIS number and plan details ahead of your first session. Once you have booked a session or contacted us, our team will be in touch to discuss our service agreement and provide you with our services price list.
